Custom action to SharePoint Ribbon

I created custom actions in SharePoint designer on Display Form Ribbon and Edit Form Ribbon. But it is not visible anywhere in the list item - default as well as custom views

Please help me create a custom action for display form ribbon that runs a workflow for that item
